[Bug] - Hand of Nostrum Level 3

In the description, Hand of Nostrum at Level 3 is supposed to add 3 Healing Beams. So the Witch Doctor is supposed to be casting 4 Beams. (At Level 2, it adds 2 Healing Beams, and the Witch Doctor is casting 3 Healing Beams on heal.)

But in game, I am only seeing 3 Healing Beams at Skill Level 3. Has there been any changes to the how this skill works?

No changes have been made to the skill, no.
Is it possible that the WD has fewer than 4 targets, so more than one beam is used on a single target, making them overlap and appear to be fewer?
